1: As goco11 says, there are card issuers available to everyone who apply zero fees and use the (as near as makes no difference) wholesale "mid" rate to calculate the exchange i.e. is totally fee-free. There is likely nothing stopping you from getting one of these. It doesn't have to come from your "own" bank. There are probably others, but Barclaycard currently have a "Rewards" Visa which, in addition to being fee-free, gives you a little cashback on your spending as well; Halifax Clarity are a couple of examples.

2: Even those cards that do charge a fee, it's most often expressed as a % of the exchange rate which is again based on that wholesale rate and is usually around 3%. You have to recognise that when you use any money exchange service to obtain hard cash, such places *never* use the wholesale rate; it's already adjusted from that by a % which is typically around 5 (varies). In other words, even with a 3% turn, you may well still be a couple of % better off. But clearly, getting and using a fee-free card is better yet.

1: Ignores market fluctuations between the time you (would have) bought your cash vs. the time you actually spend on a card. Market, despite what some might say, is totally unpredictable and will shift in your favour as many times as against it.
